Abstract

Mucosotrope humanpathogene Papillomviren (HPV) werden in zwei Gruppen eingeteilt. Die Viren der Hochrisikogruppe, vorallem die HPV-Typen 16 und 18, werden u.a. für die Entstehung von Gebärmutterhalskrebs und ca. 15 der Kopf-Hals Plattenepithelkarzinome verantwortlich gemacht. Viren der Niederrisikogruppe, vorallem HPV 6 und 11, rufen gutartige anogenitale Condylome und orale Papillome hervor. Die gesamte Virusreplikation findet in den Zellen der sich bildenden Warze statt, dabei werden sowohl die regulatorischen Proteine (E1 bis E7), als auch die Kapsidproteine (L1 und L2) des Virus exprimiert. Die Ausprägung der Läsionen ist in den Patienten – möglicherweise bedingt durch unterschiedliche Immunantwort - stark variabel. Es liegen in der Literatur Daten zur Antikörperantwort gegen die Kapsidproteine vor, während die Antwort gegen frühe Proteine mangels geigneten Untersuchungsverfahren bisher kaum untersucht wurden. In dieser Arbeit wurden sechs neue, einheitlich aufgebaute ELISA entwickelt und die Antikörperantwort in 68 gesunden Erwachsenen (Kontrollgruppe), 81 Patientinnen mit Genitalcondylomen und 101 Patienten mit rekurrierender respiratorischer Papillomatose (RRP) gegen die Kapsidproteine L1 und L2 sowie gegen die frühen regulatorischen Proteine E2, E4, E6 und E7 untersucht. Als Antigene dienten bakteriell exprimierte, lösliche GST-Fusionsproteine. Der GST-L1tag ELISA reproduzierte die Daten eines konventionellen HPV Kapsid-ELISA mit der selben Serumgruppe sehr gut (r = 0,79). Mit dem Hauptkaspidprotein L1 reagierten 31 der Kontrollgruppe, 88 der Condylomgruppe und 51 der RRP-Gruppe positiv. Die erhöhte L1 Antikörperprävalenz bei den RRP-Patienten war durch 83 L1 positive Patientinnen bedingt, während bei den männlichen Patienten mit 38 Seroprävalenz kein Unterschied zur Kontrollgruppe bestand. Translation of abstract (English)

Mucosotropic Human Papillomaviruses (HPV) are subdivided in two groups. Viruses of the high-risk group, mainly HPV16 and HPV18, which induce cervical carzinoma and 15 of the head and neck squamous cell carcinomas. Low-risk HPV induce benign genital Condyloma acuminata and oral Papillomata. The viral life-cycle takes place in the proliferating cells of the wart and all viral proteins (early proteins E1 to E7 as well as capsid proteins L1 and L2) are expressed. The strongly variable manifestations of HPV-induced lesions migth be due to differences in the hosts’ immune system. Available data in the literature focuses mainly on the L1 and L2 immune response. Only limited data on the immune response against the early proteins is available maybe due to the lack of a suitable test system. In this work 6 different ELISA of the same system were newly developed and evaluated and the immune response of a control group (n=68 individuals), patients with genital condyloma acuminata (n=81), and patients with recurrent respiratory papillomatosis (RRP, n=101) was tested. Antigens were bacterially expressed, soluble GST-fusion proteins including full length capsid proteins L1 and L2 as well as the early proteins E2, E4, E6 and E7 of HPV type 6b. The L1 ELISA reproduced the data obtained with a conventional VLP ELISA well (r=0.79). 31 of the control group, 88 of the patients with genital condylomata, and 51 of RRP patients reacted positive in the L1 ELISA. The elevated antibody prevalence in the RRP group came from 83 positive female patients whereas only 38 of the male patients were L1 ELISA positive. No statistical significant difference was obsereved between male RRP patients and male individuals of the control group. The antibody reactivities of the condyloma patients were higher than the RRP patients’ reactivities.
