Abstract

Zur Bekämpfung der Malaria, an der jährlich weltweit eine Millionen Menschen sterben, kommen auch zwei Unterklassen der Chinolin-Antimalariamittel zum Einsatz. Chloroquin, was zu den 4-Aminochinolinen gehört und die Diastereomere Chinin und Chinidin, die zu den Arylaminoalkoholen gezählt werden. Eine wichtige Rolle bei der Ausbildung der Chloroquin-Resistenz hat PfCRT (P. falciparum „chloroquine resistance transporter“). Die Chinin- und Chinidin-Resistenz scheinen jedoch weit komplexer zu sein und es wird vermutet, dass mehrere Faktoren hieran beteiligt sind. Diskutiert werden PfCRT, PfMDR1 (P. falciparum „multi drug resistance 1“) und PfNHE (Natrium-Protonen-Austauscher). In dieser Arbeit wurde die initiale [3H]-Chloroquin- und [3H]-Chinidin-Akkumulation in den Stämmen der genetischen Kreuzung aus HB3 und Dd2 und ihren F1-Nachkommen gemessen und anschließend eine QTL-Analyse („Quantitative Trait Locus”) durchgeführt. Mit der Bestimmung der initialen Akkumulationen wurden lediglich Transport- und Partitionierungsprozesse über die Membran bestimmt. Während die Chloroquin-Akkumulation hauptsächlich von PfCRT beeinflusst war, wurde die Chinidin-Akkumulation zwar auch von PfCRT, aber auch von einem zweiten Faktor auf Chromosom 7, dem B5M12-Lokus kontrolliert. Dieser wurde auch schon bei der Chinin-Akkumulation beschrieben. B5M12 und PfCRT interagierten dabei synergistisch. Zwei weitere Faktoren auf Chromosom 6 und 13 verändern ebenfalls die Chinidin-Akkumulation. Der Lokus auf Chromosom 13 entsprach nicht PfNHE. Um dennoch den Einfluss von PfNHE auf die Chinin- und Chinidin-Akkumulation zu bestimmen, wurden Akkumulationsexperimente mit drei Austauschmutanten mit verminderter PfNHE-Expression durchgeführt. Ein Einfluss von PfNHE auf die Akkumulation konnte mit diesen Experimenten ausgeschlossen werden. In der vorliegenden Arbeit konnte der B5M12-Lokus von 36 auf 25 Kandidatengene eingeschränkt werden. Dabei konnten Gene ohne Sequenzunterschiede zwischen HB3 und Dd2 ausgeschlossen und der genaue Lokusbereich durch Bestimmung der „Crossing-over“-Region eingeschränkt werden. Eine Regression der Chinin- und der Chinidin-Akkumulation zeigte, dass die Akkumulationen beider Medikamente positiv linear korreliert sind. Die Hauptfaktoren der Akkumulation beider Medikamente scheinen somit übereinzustimmen.

Translation of abstract (English)

Annually, one million people are dying from Malaria. For the treatment of this disease two subclasses of the quinoline antimalarials are widely used. Chloroquine belongs to the subclass of the 4-aminoquinolines, whereas the diastereomers quinine and quinidine are members of the arylamino alcohols. PfCRT (P. falciparum chloroquine resistance transporter) has an important role in mediating chloroquine resistance. However, the mechanisms underlying quinine and quinidine resistance are not so easily explained. A multifactorial mechanismus is proposed. Considered are PfCRT, PfMDR1 (P. falciparum multidrug resistance 1) and PfNHE (putative sodium/hydrogen exchanger). In this study, the initial accumulation levels of [3H]-chloroquine and [3H]-quinidine in the progeny of the HB3 x Dd2 genetic cross were investigated and a QTL-analysis (quantitative trait locus) was performed. The rationale behind these experiments is that only facilitating, transporting and partitioning processes are being taken into account. Whereas chloroquine accumulation was mainly under control of PfCRT, quinidine accumulation was affected by PFCRT and a second factor on Chromosome 7, the B5M12 locus. B5M12 and PfCRT interacted synergistically. Two minor factors on chromosomes 6 and 13 played also a role in the quinidine accumulation. The locus on chromosome 13 did not correspond to PfNHE. This was shown by measuring the direct contribution of PfNHE to quinidine and quinine accumulation in experiments with PfNHE allelic exchange mutants. A correlation between PfNHE and quinidine and quinine accumulation could not be detected. In this study the 36 candidate genes found in the B5M12 locus could be reduced to 25 genes by excluding genes having no polymorphisms between HB3 and Dd2 and by narrowing the crossing over regions. A regression between quinine and quinidine accumulation showed that both drugs are positively correlated which means that quinine and quinidine accumulation share the same main factors.
