Abstract

Die Magnetoenzephalographie (MEG) ist eine Methode zur nichtinvasiven Lokalisierung zerebraler Hirnaktivität. Sie kann daher sehr gut für eine prächirurgische Diagnostik eingesetzt werden, um Vorhersagen zu treffen, inwieweit pathologische Gewebeveränderungen - wie Tumore - nahe an entscheidende Areale spezieller Wahrnehmungsmodalitäten heranreichen. Die ermittelten kortikalen Aktivitätszentren müssen in bildgebende Verfahren, wie z.B. MRT, projiziert werden, was als MEG-MRT-Koregistrierung bezeichnet wird. Hier wird ein Verfahren zur Koregistrierung vorgestellt, welches auf einer computerunterstützten Abstandsminimierung beruht. Zur Ermittlung kortikaler Aktivität somatosensibler Bereiche des Körpers müssen die entsprechenden Stellen, wie Finger oder Lippe, spezifisch gereizt werden. Hier stehen zwei Methoden zur Auswahl; elektrische Reizung afferenter Nervenbahnen oder taktile Stimulation der Hautbereiche. Da die elektrische Reizung Nachteile aufweist, wird in dieser Arbeit die erfolgreiche Entwicklung taktiler Stimulation vorgestellt. Ein weiteres nicht invasives Verfahren zur Bestimmung zerebraler Hirnaktivität, die funktionelle Magnetresonanztomographie (fMRT), beruht auf der Messung von Blutfluß- und Blutvolumenänderungen. Wünschenswert für eine valide Diagnostik ist die Messung an Patienten sowohl mit MEG als auch fMRT. In dieser Arbeit werden Studien vorgestellt, bei denen an einem Probandenkollektiv der somatosensorische Kortex mit beiden Meßverfahren lokalisiert wurde. Der Vergleich der Daten erlaubt zu untersuchen, ob sich gleiche Lokalisationen ergeben müssen, oder aufgrund der unterschiedlichen Verfahren grundsätzliche Unterschiede aufkommen. Die Ergebnisse der Arbeit konnten für eine prächirurgische Diagnose genutzt werden, indem taktile Messungen an Patienten mit intrakraniellen Tumoren durchgeführt wurden.

Translation of abstract (English)

Magnetoencephalography (MEG) allows noninvasive studies of human cortical areas. It can be used as a clinical tool for presurgical diagnostics to ascertain whether pathological changes of tissue already infiltrate cortical areas of perception. Cortical centres of activity obtained by MEG have to be projected into morphological images like MRI. This procedure is called MEG-MRI-coregistration. In order to minimize possible errors, which can severely impair the accuracy of localizing cortical centres of activity, a coregistration method is presented, which is based on a fully automatic minimization of distances of two point-sets. In order to localize cortical activity of somatosensory perception, specific areas of the human body have to be stimulated. This can be done either by electric or by tactile stimulation. Although well established, electrical stimulation comprises some disadvantages. Therefore the successful development and optimization of tactile stimulation of hand and lip area, including appropriate data analysis is presented in this thesis. Another noninvasive technique of localizing cerebral activity, the functional Magnetic Resonance Imaging (fMRI), is based on measuring specific changes in blood flow and blood volume. For presurgical localization MEG and fMRI should be combined to improve the efficiency of prognosis. Therefore studies with a group of probationers were performed in order to localize the somatosensory cortex with both techniques. By comparing the located centres of activity it is possible to investigate principal differences, which might lead to divergent results of the two methods. The results of the thesis were used to perform first presurgical diagnosis by means of tactile stimulation on patients with intracranial tumors.
