Abstract

In dieser Arbeit wurde ein neuartiger aktiver Katheter mit auffaltbarer Spule für die hochaufgelöste intravaskuläre MR-Bildgebung entwickelt. Dazu wurde eine Spule auf eine flexible Trägerfolie aufgebracht, die im Gefäß aufgefaltet wird, um die Fläche und damit die Eindringtiefe der Spule zu erhöhen. Zur Verringerung von Bewegungsartefakten aufgrund des pulsatilen arteriellen Blutstroms wurde eine neuartige Bewegungskorrekturmethode entwickelt, die eine MR-Bildgebungssequenz mit der Akquisition von Projektionsdaten zur Lokalisation der Katheterspule kombiniert. Anhand der Projektionsaufnahmen wurde ein retrospektives Gating der Bilddaten durchgeführt. Die Funktionalität des Katheters und die Effizienz der Bewegungskorrekturmethode wurden in Phantom- und Tierexperimenten evaluiert. Mithilfe der Bewegungskorrektur ließ sich ein um bis zu 570% höheres SNR im Vergleich zu den nicht korrigierten Aufnahmen erzielen und die Darstellung anatomischer Strukturen mit einer Größe von 150 µm erreichen. Des Weiteren wurden MR-Bildgebungsmethoden zur Echtzeitvisualisierung transarterieller Embolisationen entwickelt. Von besonderer Wichtigkeit ist dabei die korrekte Bestimmung des Embolisationsendpunktes, um einen Reflux des Embolisats in andere Gefäße zu verhindern. Mithilfe einer speziellen Saturation-Recovery-TurboFLASH-Sequenz konnten erstmals vollständig MR-geführte transarterielle Embolisationen an einem Tiermodell (Schwein) durchgeführt werden. Die Verteilung eines intraarteriell applizierten Kontrastmittels wurde dabei mit einer Zeitauflösung von 2 Bildern pro Sekunde dargestellt. Durch die Embolisation wurde die Perfusion um bis zu 90% reduziert, ohne dass ein Reflux des Embolisationsmaterials beobachtet wurde.

Translation of abstract (English)

In this work a novel active catheter with an expandable coil for high-resolution intravascular MR imaging was developed. The coil was placed on a flexible foil carrier, which was unfolded in the artery to increase the coil area and therefore to enhance the penetration depth of the coil. To reduce motion artifacts due to the pulsatile blood flow, a novel motion correction method was developed that combines an MR imaging sequence with the acquisition of projections for a retrospective gating of the image data. The functionality of the catheter and the efficiency of the correction method were evaluated in phantom and animal experiments. The SNR of the corrected images was increased by up to 570% over the uncorrected images, and anatomical structures of 150 µm size could be differentiated in the aorta. Furthermore MR imaging methods for real-time monitoring of transarterial embolizations were developed. The precise definition of the embolization endpoint is essential to avoid over-embolization. With a special saturation recovery turboFLASH pulse sequence, MRguided transarterial embolizations could be performed successfully in domestic pigs. The distribution of an intraarterially applicated contrast agent was visualized with a temporal resolution of 2 images per second. The perfusion rate could be reduced by up to 90% while a reflux of embolization material was not observed.
