Abstract

Der Einsatz mobiler Kleinstcomputer in der klinischen Routine besitzt großes Potential, die Kommunikations- und Informationsbedürfnisse von Ärzten und Pflegern zeitnah an deren unterschiedlichen Wirkungsstätten (z.B. am Patientenbett) zu erfüllen. Obwohl zahlreiche Arten von leistungsfähigen, tragbaren Kleinstcomputern zur Verfügung stehen, setzen sich diese nur langsam als Werkzeuge der Informationsverarbeitung in Krankenhäusern durch. Ziel dieser Arbeit ist es, die Gestaltung von mobilen Informations- und Kommunikationswerkzeugen in Bezug auf Funktionalität, Geräteeigenschaften und Integrationsmöglichkeiten in heterogene Krankenhausinformationssysteme (KIS) zu erarbeiten. Dabei werden die Wünsche der klinischen Anwender und die derzeitige Praktikabilität berücksichtigt. Zum Erreichen dieses Zieles wurden zwei praxisnahe Evaluationen am Universitätsklinikum Heidelberg durchgeführt. Anhand internationaler Literatur und aus diesen Ergebnissen werden wesentliche Integrationsaspekte abgeleitet und in einen Leitfaden für die Integration übertragen. Die Anwendbarkeit dieses Leitfadens wird anhand eines Realisierungsvorschlages für das Universitätsklinikum Heidelberg demonstriert. Insgesamt zeigt sich, daß das wichtigste mobil zu realisierende Verfahren die Unterstützung der interpersonellen Kommunikation ist. Weitere wichtige Verfahren sind z.B. die Einsicht in eine 'Elektronische Patientenakte', 'Klinische Dokumentation', 'Koordination mit Leistungserbringern', 'Anfordern von Essen, Material und Medikamenten' und der 'Zugriff auf Wissen'. Die funktionale Überschneidung zu festinstallierten Klinischen Arbeitsplatzsystemen (KLAPS) ist offensichtlich. Deutlich wird, daß mobile Kleinstcomputer die Benutzung von KLAPS räumlich ausweiten und die Anzahl der fest zu installierenden KLAPS verringern, jedoch diese nicht ersetzen können. Sie müssen vielmehr an die örtlichen Gegebenheiten, Organisationsstrukturen und Arbeitsweisen unterschiedlicher Personengruppen angepaßt ein

Translation of abstract (English)

The use of small mobile computers in clinical routine has a great potential to fulfil communication and information needs of healthcare professionals at different points of care (e.g. at patient's bedside, on ward). Although different powerful mobile computers exist, up to now they are rarely used in clinical settings. Aim of this thesis is to work out the design for mobile information tools considering necessary functionality, hardware requirements and possibilities of their integration into heterogeneous hospital information systems. Hereby the health care professionals demands and the present practicability are taken into account. To reach this aim two close to reality studies were carried out at Heidelberg University Hospital. Based on international literature and the results of these studies a guide(line) for the integration of mobile information and communication tools into hospital information systems is developed. The applicability of this guide(line) is demonstrated with the help of a suggestion for realizing the integration of mobile tools into the Heidelberg Hospital Information System. Altogether it becomes evident that the most important mobile function is the support of interpersonal communication. Further important functions are for example 'to have a look into an electronic patient record', 'clinical documentation', 'ordering of food, material and drugs' and the 'access to knowledge'. The functional overlapping with health professionals workstations is obvious. It shows that the use of mobile computers can extend the use of health professional workstations and might reduce the number of needed fix installed workstations. Nevertheless mobile workstations can not replace them. Mobile computers must rather be used in addition and be very much adapted to local conditions, organizational structures and the different ways of working of different groups of people.
