Abstract

In dieser ersten Effektivitätsstudie zur psychotherapeutischen Beratung Studierender im deutschsprachigen Raum, wurde bei 151 KlientInnen die Wirksamkeit eines integrativen Beratungskonzepts untersucht.

Das integrative Beratungskonzept umfasst supportive (Alliance), verhaltensorientierte (Behavior), kognitive (Cognitions), psychodynamische (Dynamics) und existentielle Elemente (Existentials) und lässt sich zusammenfassend als ABCDE-Modell beschreiben. Es erlaubt eine Betrachtung der gegenwärtigen individuellen Problematik des Klienten aus unterschiedlichen und sich wechselseitig ergänzenden Perspektiven und gewährleistet ein umfassendes Verständnis menschlichen Erlebens, Denkens, Handelns und Fühlens und darauf bezogener Einschränkungen. Auf Basis dieses Verständnisses erfolgt die Wahl geeigneter Strategien für die Auseinandersetzung mit der aktuellen Problematik und deren Bewältigung.

Auch im Vergleich zu den bisherigen internationalen Studien in diesem Anwendungsfeld ist die Effektivität psychotherapeutischer Beratung nach dem ABCDE-Modell als hoch einzustufen. Die Prä-Post-Veränderungen auf den Symptom-, Zufriedenheits- und Kreativitätsmaßen waren signifikant und erreichten überwiegend mittlere bis hohe Effektstärken. Ein hoher Prozentsatz an KlientInnen verbesserte sich im Beratungsverlauf statistisch und auch klinisch signifikant.

Die Ergebnisse der Dosis-Wirkungsanalysen legen nahe, dass für eine statistisch signifikante Verbesserung der Hälfte der KlientInnen mindestens acht und für eine klinisch signifikante Verbesserung mindestens neun Sitzungen notwendig sind und in der Bedarfsplanung entsprechend berücksichtigt werden sollten.

Die im Unterschied zu den bisherigen Effektivitätsstudien erfolgte Miteinbeziehung der Fremdbeurteilungssicht und die Diagnostik nach ICD-10 verdeutlichen, dass für das Beratungsangebot geeignete KlientInnen weiterbehandelt werden, während psychisch und in diagnostischer Hinsicht stärker beeinträchtigte KlientInnen weitervermittelt werden, die Beratung abbrechen oder den Post-Bogen nicht ausfüllen.

Die vorliegende Arbeit stellt einen wichtigen Beitrag zur Ergebnisforschung im Bereich psychotherapeutischer Beratung für Studierende dar. Über die Verwendung breiter und dem Gegenstandsbereich angemessener Ergebniskriterien ergibt sich aus unterschiedlichen Beurteilungsperspektiven ein differenziertes Bild von (individueller) Veränderung im Verlauf integrativer psychotherapeutischer Beratung. Einschränkungen der vorliegenden Untersuchung betreffen die insbesondere durch das Fehlen einer Kontrollgruppe bedingte geringe interne Validität.

Translation of abstract (English)

In this first effectiveness study of psychotherapeutic counseling for students in German-speaking countries, the effectiveness of an integrative model of counseling was evaluated based on a sample of 151 clients.

The integrative model of counseling contains supportive (alliance), behavioral (behavior), cognitive (cognitions), psychodynamic (dynamics) and existential elements (existentials) – therefore it can be called ABCDE model of counseling. It allows consideration of the client’s current concerns and problems from different and mutually complementary theoretical perspectives, and it provides a comprehensive understanding of human experience, thinking, feeling and acting as well as associated restrictions. Based on this understanding, the appropriate strategies to work with and to overcome the current set of problems are chosen.

Effectiveness of integrative counseling according to the ABCDE model was found to be high in comparison to other international studies in this area. Pre-post differences on measures of mental distress, satisfaction and creativity were significant and effect sizes were mostly moderate to high. A high percentage of clients improved statistically and clinically significant.

Results from dose-response analyses indicated that at least eight sessions were needed to attain statistically significant improvement for half of the clients, while clinically significant improvement required a minimum of nine sessions. These results should be taken into consideration for the planning of demand.

Counselors’ expert ratings and diagnostics according to ICD-10 that have been included in contrast to previous effectiveness studies showed that clients suitable for the counseling setting get treated in the counseling center while more severely disturbed clients in terms of psychopathology or diagnosis get referred to outpatient treatment, drop out or object to provide post data.

This study represents an important contribution to effectiveness research in the field of psychotherapeutic counseling for students. Through the use of broad and appropriate criteria of success for this specific field of science and a combination of assessments from different perspectives, a more sophisticated understanding of (individual) change over the course of integrative psychotherapeutic counseling was achieved. A limiting factor of this study is the low internal validity due to the lack of a control group.
