"Our knowledge on the population of exoplanets and circumstellar disks has increased drastically\r\nin the last decades. Yet many processes during the formation of planets, especially in the\r\nintermediate size range, are unobservable. This thesis focuses on linking the evolution of a\r\ncircumstellar disk with a final set of planets in a globally self consistent framework. The number\r\nof embryos and their formation in current planet formation models are subject to assumptions and\r\nnot to physical modeling. This inconsistency currently marks the single largest blind spot in global\r\nplanet formation modeling. Within four consecutive publications, I present key improvements\r\nin global planet formation modeling. Namely the evolution of dust and pebbles, the formation\r\nof planetesimals and the formation of planetary embryos. Within this thesis I present a global\r\nplanet formation model that self-consistently tracks the formation of planets from an initial\r\ndisk of gas and dust during its entire lifetime. For the first time, this is achieved without far\r\nreaching assumptions on initially placed planetesimals or planetary embryos. I show that the\r\ndisk consistent treatment of planetary embryo formation results in multiple distinct embryo\r\ngenerations during the lifetime of the circumstellar disk. A clear dichotomy between planets that\r\nform in different generations is found.
