Abstract

Patienten mit visuell-räumlichen Neglektsymptomen beachten Reize der kontraläsionalen Raum- und/oder Körperhälfte nicht. Diese Vernachlässigung kann auch in der mentalen Repräsentation auftreten. Mentales Training wird vermehrt im rehabilitativen Setting als Komplement zur Physiotherapie eingesetzt. Ziel der vorliegenden Arbeit war herauszufinden, ob eine visuomotorische Imaginationstherapie bei Neglektpatienten in unterschiedlichen Krankheitsstadien durchführbar ist und anhaltende sowie generalisierbare Veränderungen sowohl in der Neglektsymptomatik wie auch in der Körperwahrnehmung herbeiführen kann. Im Rahmen der visuomotorischen Imaginationstherapie wurden Positionen und sequentielle Abläufe des kontraläsionalen Armes wiederholt so lebendig und intensiv wie möglich mental geübt. Detaillierte Instruktionen und ein adaptives Vorgehen sollten eine intensive kinästhetische Imagination gewährleisten. Die Machbarkeit wurde anhand von Therapieprotokollen und elektromyographischen Ableitungen erfasst. Die Effektivität der Imaginationstherapie wurde anhand einer ausführlichen neuropsychologischen Testbatterie überprüft. Fünfzehn subakute Neglektpatienten der Kliniken Schmieder Heidelberg erhielten eine dreiwöchige Imaginationstherapie als Add-On-Verfahren und wurden mit fünfzehn Neglektpatienten verglichen, die keine zusätzliche Therapie zur neurologischen Rehabilitationsroutine erhielten. Zehn Patienten mit chronifiziertem Neglekt nahmen an einer vierwöchigen Einzeltherapie im Rahmen eines Baseline-Designs mit katamnestischer Nachuntersuchung teil. Zum eigenständigen Weitertrainieren wurde den Patienten eine Audio-CD mit Instruktionen für visuomotorische Imaginationsübungen ausgehändigt. Bei neun chronischen Neglektpatienten wurde vor der Interventionsphase eine funktionell-bildgebende Untersuchung durchgeführt zur Erfassung der generellen Imaginationsfähigkeit bei Neglekt. Die Therapieprotokolle wiesen auf eine hohe Compliance und subjektive Gewinne in der Wahrnehmung der linken Raum- und Körperhälfte bei allen Patienten hin. Kinästhetische Imagination war von sensorischen Empfindungen und muskulärer Aktiviät begleitet. Bei der Imagination von Bewegungen der betroffenen Hand wurden verstärkt kortikale Areale der kontraläsionalen Hemisphäre rekrutiert. Proportional zur Eingangsleistung verbesserten sich die subakuten Neglektpatienten im Vergleich zur Kontrollgruppe deutlich mehr beim Abzeichnen, in der Körperwahrnehmung und der Armsensorik sowie –motorik. Signifikante anhaltende therapieinduzierte Verbesserungen zeigten sich in der Exploration, der kinästhetischen Imagination und Alltagsleistungen. Das Eigentraining wurde als weniger intensiv empfunden als die Therapie mit persönlicher Assistenz. Visuomotorische Imagination ist durchführbar bei Neglektpatienten im subakuten und chronischem Krankheitsstadium. Schmerzen und depressive Verstimmungen stellten Kontraindikatoren dar. Perspektivisch sollen psychometrische Instrumente zur Beurteilung der individuellen Imaginationsfähigkeit, Ein- und Ausschlußkriterien für die Patientenauswahl, Methoden zur statistischen Effektivitätsüberprüfung der visuomotorischen Imagination, sowie Eigentrainingstechniken ausgearbeitet werden.

Translation of abstract (English)

Patients with visuospatial neglect are unaware of stimuli on the contralesional half of body and/or space. Neglect can also affect the mental representation. Mental practice is increasingly applied in the rehabilitative setting as a complement to physiotherapy. The aim of this study was to investigate the feasibility of a visuomotor imagery therapy in neglect patients at different times after stroke and the efficacy of the approach in modifying visuospatial and representational neglect symptoms. During visuomotor imagery therapy positions and movements of the contralesional upper limb were repetitively imagined as vividly and intensively as possible. Detailed instructions and an adaptive procedure were applied to induce an intensive kinaesthetic imagery. Feasibility was assessed using verbal-feedback protocols and electromyographic recordings. Efficacy was determined using various standardized functional-outcome measures and neuropsychological tests. Fifteen subacute neglect patients of the Kliniken Schmieder Heidelberg received two daily half-hour sessions of visuomotor imagery therapy over a three-week period as an add-on treatment and were compared to a control group of 15 subacute neglect patients without supplementary therapy. Ten patients with chronic neglect symptoms took part in a four weeks’ imagery intervention within a baseline-design with a catamnestic follow-up examination. An audio-CD with instructions for visuomotor imagery exercises was handed out to the patients for individual training. Nine chronic neglect patients were included in an fMRI-study before the intervention period to define neuronal correlates of their general imagery capacity. According to therapy protocols, compliance and subjective-gain ratings of increased left-side awareness were high. Kinaesthetic imagery was accompanied by a change in muscular-activity patterns. During imagery of the affected arm cortical areas of the contralesional hemisphere were increasingly recruited. Sucacute neglect patients of the intervention group improved more proportionally to the pre-measurement performance as compared to the control group in drawing tasks, body awareness and arm sensory as well as motor functions. Significant enduring therapy-induced improvements were acquired in exploration, kinaesthetic imagery and ADL. The individual training was perceived as less effective than the therapy with personal assistance. Visuomotor imagery is applicable with neglect patients in the subacute and chronic phase after stroke. Pain and depressive moods are contraindications. Perspectively, psychometric instruments to assess individual imagery capacity, in- and exclusion criteria for patient selection, methods for the statistical evaluation of the efficacy of visuomotor imagery, as well as techniques for individual training should be elaborated.
