Abstract

Die Forschungsarbeit setzt sich aus zwei Studien zusammen: einer klinischen Untersuchung einer Stichprobe depressiver Patienten in stationärer psychosomatisch-psychotherapeutischer Behandlung, deren zentrales Thema die Validierung der Operationalisierten Psychodynamischen Diagnostik (OPD-2)darstellt, sowie einer interkulturellen Vergleichsstudie in welcher der Einfluss interkultureller Variablen auf die Unterschiede zwischen Deutschen und Chilenen bezüglich der Ausprägung von Depressivität untersucht wurde. Beiden Studien ist der Einsatz des Depressive Experience Questionnaire (DEQ; Blatt et al., 1976) gemeinsam, der die Erfassung der anaklitischen versus introjektiven Depression als traditionelle Subtypen depressiver Persönlichkeitsentwicklung vornimmt und somit eine Betrachtung depressiver Erkrankungen über die symptombezogenen Maße hinaus und vor dem Hintergrund eines psychodynamischen Krankheitsverständnisses ermöglicht. Bei der klinischen Untersuchung handelt es sich um Psychotherapieforschung im stationären Setting mit medizinischer und psychotherapeutischer Versorgung, eine naturalistische, prospektive Beobachtungsstudie ohne Kontrollgruppe, welche differentielle Therapieeffekte der psychodynamisch orientierten stationären Psychotherapie bei depressiven Patienten erforscht. Auf der Grundlage der Konfliktachse der OPD-2 wurde an 72 stationären depressiven Patienten empirisch untersucht, ob sich die von Blatt (1974)postulierte Unterteilung in Depressive vom introjektiven und anaklitischen Subtyp abbilden lässt. Weiterführend wurde geprüft, ob signifikante Unterschiede der Therapieeffekte auf Symptomebene zwischen Patienten unterschiedlicher Subtypen, beziehungsweise unterschiedlicher intrapsychischer Konflikte nach OPD-2, verzeichnet werden können. Der interkulturelle Aspekt der Forschungsarbeit besteht in dem Einsatz der adaptierten Version des DEQ für Chile neben dem BDI als bekanntem symptombezogenem Maß und unter Berücksichtigung der kulturellen Variable des Selbstkonzepts. Mittels einer parallelen Stichprobenrekrutierung an der Allgemeinbevölkerung in Deutschland und in Chile wurde es ermöglicht, eine erste explorative Untersuchung zum Einfluss dieser bedeutenden Kulturvariable in Zusammenhang mit dem traditionellen Subtypenkonzept zu betrachten.

Translation of abstract (English)

The dissertation project contains a clinical and an intercultural study. Background: Depressive disorders rank among the most prevalent mental disorders (American Psychiatric Association, 2000). Therefore, a reliable and clinical valid diagnostic has become indispensable. While symptom-oriented classification systems (e.g. DSM-IV; ICD-10) can only partially guide therapeutic interventions for psychodynamic oriented psychotherapists, the Operationalised Psychodynamic Diagnostic (OPD-2) (Arbeitskreis OPD, 2006) extends the existing manuals for classification of psychic disorders by including essential psychodynamic constructs. The objective of this study was to identify conflict patterns of depressive patients using the OPD-2. Another aim was to discuss a typification of depression relevant for prognosis and therapeutic interventions in order to discriminate from established theories of depressive subtypes (e.g. Arieti & Bemporad, 1978; Blatt et al., 1974). Method: To analyse the validity of the OPD-2 conflict axis, OPD-interviews taped on video and data from questionnaires filled in by depressive inpatients in psychosomatic-psychotherapeutic treatment are evaluated (n=72). Results: The most frequently diagnosed main-conflicts by OPD-2 in our sample of depressive inpatients are the Desire for Care versus Autarchy and the Self-Value Conflict. On the DEQ-Scales no significant differences result between conflict groups. In fact, patients with a passive mode of conflict processing seem to be more self-critical than patients with an active mode. The outcome data demonstrated significantly inferior therapeutic effects of patients with the conflict Dependence vs. Autonomy. Discussion: Literature shows the important role of the constructs dependency and low self-esteem as vulnerability factors of depressive disorders (Blatt & Zuroff, 1992; Coyne & Whiffen, 1995). The ratings of the main-conflicts by OPD-2 support this conclusion. Subtypes exclusively based on OPD-conflicts seem to be unsuitable with respect to the outcome criteria. There are indications that the processing mode as a variable will be important. The intercultural study focuses on the constructs dependency and self-criticism as vulnerability factors of depressive disorders by comparing non-clinical samples of Germany and Chile. In addition to the BDI, which is a well known questionnaire for symptoms of depression, and the consideration of the cultural variables independent and interdependent self-construal, an adapted version of the DEQ for Chile is introduced. By these means it is possible to conduct a first explorative investigation of the influence of self-construal in relation to the concept of traditional depressive subtypes. The most important finding is the moderator effect of the cultural variable self-construal, which causes significant intercultural differences in the anaclitic subtype.
