Abstract
Diese Arbeit unterbreitet den Vorschlag, bei der Planung und routinemäßigen Evaluation psychotherapeutischer Behandlungen innerhalb eines Qualitätsmanagements (QM) die Entwicklung des frühen Therapieverlaufs mit einzubeziehen. Die Abwägung der Erfolgschancen der Patienten kann mit Hilfe der Hinzunahme des Konzeptes des 'Ansprechens auf Therapie' bzw. Therapie-Response weiter präzisiert werden, so daß Entscheidungen für den Fortgang der Therapie auf eine sicherere Basis gestellt werden können. Die Arbeit erläutert das Konzept des Therapie-Response und ordnet ihn in bestehende, schulenübergreifende Psychotherapiemodelle ein. Am Beispiel von 870 Therapieverläufen (von Patienten aus der multizentrischen Studie zu Aufwand und Ergebnis psychodynamischer stationärer Behandlung bei Eßstörungen, MZ-ESS) werden verschiedene Operationalisierungen für Therapie-Response hinsichtlich ihrer prädiktiven Validität für Therapieverlauf und -ergebnis untersucht. In einem abschließenden Teil wird die Anwendung von Therapie-Response innerhalb eines verlaufsorientierten QM-System demonstriert. In diesem System wird die aktuelle symptomatische Entwicklung eines sich in Behandlung befindlichen Patienten mit einem Referenzverlauf verglichen und auf dieser Grundlage eine individuelle Prognose erstellt, die den Therapeuten bei der Planung der weiteren Behandlung unterstützt. In dieser Arbeit werden die Kriterien und Methoden (Hierarchische Lineare Modelle) zur Konstruktion dafür geeigneter Verlaufsstandards entwickelt und die Rolle des Therapie-Response bei der Prognose des Therapieergebnisses diskutiert. Im Ergebnis zeigte sich, daß Prognosemodelle, die die frühe symptomatische Entwicklung (Therapie-Response) berücksichtigten, bei der Vorhersage des Therapieergebnisses deutlich besser abschnitten, als Modelle, die zu Vorhersage nur die Anfangsinformation enthielten. Aus den Referenzverläufen abgeleitete Prognose-Indikatoren konnten die Vorhersage zusätzlich verbessern.
Translation of abstract (English)
Recent developments in quality management for psychotherapy favor the continuous monitoring of patient progress to decide if treatment is working or not. This decision needs valid prognoses about treatment outcome. Because early developments during treatment can enhance the prognosis of treatment outcome, the implementation of early treatment response into a quality management (QM) system should provide therapists with valid information about treatment course and outcome. This paper presents definition criteria for early treatment response and tests it's the predictive validity in a sample of 870 patients with an eating disorder in inpatient treatment (from the German multi-center study on treatment and outcome of eating disordered patients in psychodynamically orientated inpatient treatment, Project TR-EAT). Differences in course over time of early responders and non-responders are examined. The application in a QM system is demonstrated by constructing reference courses for both groups using hierarchical linear modeling. Comparing the actual symptomatic development of a currently treated patient with the reference course can be used to enhance the individual prognosis about treatment outcome. This paper presents the criteria and methods how to construct the reference courses. Early responders had a higher chance of positive outcome and the average course of responders and non-responders differed. The implementation of early treatment response in QM provided valid additional prognostic information. Implications for QM procedures and treatment planning are discussed.
