Abstract

Zur Verbesserung der Diagnostik psychischer Störungen wurden Interviewleitfäden entwickelt, mit deren Hilfe eine Klassifikation nach DSM oder ICD vorgenommen werden kann. Bei dem strukturierten Interview F-DIPS (Diagnostisches Interview bei psychischen Störungen- Forschungsversion für DSM-IV von Margraf et al.) handelt es sich um ein Verfahren, mit dem psychotherapierelevante Achse I- Störungen erhoben werden können. In der vorliegenden Studie wurden die Retest-Reliabilität und die Validität des Instruments untersucht sowie Faktoren, die die Reliabilität beeinflussen. Eine genaue Fehleranalyse wurde durchgeführt. 191 Patienten aus Universitätskliniken (psychosomatisch und psychiatrisch) und einer psychosomatischen Rehabilitationsklinik wurden einem F-DIPS im zeitlichen Abstand von 2 Wochen unterzogen. Die Validität wurde durch gut erprobte Fragebögen und anhand der klinischen Diagnose geprüft. Insgesamt zeigte sich eine ausgezeichnete Reliabilität bei den Essstörungen (Kappa 0,89). Eine gute Reliabilität ergab sich beim Substanzgebrauch (k = .70), der Major Depression (k = .72), bei bipolaren Störungen (k = .83) und der Hypochondrie (k = .75). Eine ungenügende Übereinstimmung zeigte sich bei der GAS (k = .36, Yules-Y = .61) und der Dysthymie (k = .31, Y = .51). Bei Angst- und somatoformen Störungen zeigte sich die Übereinstimmung von zwei Interviewern besonders abhängig vom Vorliegen einer Cluster-B-Persönlichkeitsstörung und einer hohen Komorbidität. Das F-DIPS misst sämtliche Störungen valide. Insgesamt kann in Anbetracht der unselektierten, komorbid gestörten Klinik-Stichprobe bei klinisch relativ gering erfahrenen Interviewern davon ausgegangen werden, dass sich das F-DIPS sehr gut für affektive Störungen, Substanzgebrauch und Essstörungen eignet, um in diagnostisch schwierigen Stichproben eingesetzt zu werden. Bei hoher Komorbidität sollten die Diagnosen von Angst- und somatoformen Störungen z.B. mit zusätzlichen Fragebögen erhärtet werden.

Translation of abstract (English)

To improve the diagnostic of psychiatric disorders several structured and standardized interviews have been developed for a classification according to DSM or ICD. The fully structured interview F-DIPS (Diagnostic interview for psychiatric disorders – research version, Margraf et al.) is such an instrument. With the F-DIPS it is possible to diagnose Axis I disorders relevant for psychotherapy. The study analyses the retest reliability and deals with the validation of the instrument. Factors of influence for the reliability are examined and an error analysis for non-agreement is performed. 191 patients of two university hospitals (psychosomatic and psychiatric) and a psychosomatic rehabilitation hospital had two F-DIPS-interviews in a two weeks interval. The validation was done with well tested questionnaires and with clinical diagnoses. There is an almost perfect reliability for eating disorders (kappa 0,89) and a substantial reliability for substance disorders ((k = .70), Major Depression (k = .72), bipolar disorders (k = .83) and hypochondria (k = .75). An only fair agreement was found for the GAD(k = .36, Yules-Y = .61) and for the dysthymia (k = .31, Y = .51). The agreement between two interviewers depended on the existence of a personality disorder from the impulsive type and on a high comorbidity. In particular there was a negative influence in anxiety disorders and somatoform disorders. The validity of the F-DIPS is good for all disorders. Considering the difficult conditions with an unselected hospitalised sample with many comorbid disorders, rateded by interviewers with little clinical experience, the F-DIPS proves to be a useful instrument for diagnosing affective, substance and eating disorders also for diagnostic difficult samples. If there is a high comorbidity the diagnostic results for anxiety and somatoform disorders should e.g.be validated with questionnaires results.
