Abstract

Cytochrom P450 Enzyme stellen die wichtigsten am Arzneimittelmetabolismus beteiligten Enzyme in menschlicher Leber dar. Ihre Expression und Funktion kann von biologischen, genetischen und Umweltfaktoren abhängen. Der Einfluss dieser Parameter wurde in dieser Arbeit für das bisher wenig untersuchte Cytochrom P450 Enzym CYP2B6 analysiert. Die Expression und Funktion wurde an einer humanen Leberbank mit über 100 Proben untersucht. Die mittels Immunoblot quantifizierte CYP2B6 Proteinexpression variierte 191fach (0,5 bis 95,5 pmol/mg). Als Grundlage für funktionelle Untersuchungen wurden zunächst zwei postulierte Modellsubstrate für CYP2B6 untersucht und miteinander verglichen. Die Ergebnisse zeigten, dass Bupropion das spezifischere Substrat darstellt. Als eine Ursache der variablen Expression und Funktion dieses Enzyms waren verschiedene Mutationen im CYP2B6-Gen identifiziert worden. In Leberproben, als auch mit rekombinant exprimierten Proteinvarianten konnten die relativ häufig vorkommenden varianten Allele *5, *6 und *15 mit einer signifikant verringerten Expression und Funktion von CYP2B6 im Vergleich zur Wildtypgruppe (*1/*1) in Zusammenhang gebracht werden. Für weitere, seltenere Mutationen im CYP2B6-Gen konnte gezeigt werden, dass diese sogar zu einer fehlenden Expression und Funktion von CYP2B6 führen. Zusätzlich konnte als Ursache für verringerte Enzymaktivitäten nicht nur eine verminderte Proteinexpression, sondern auch eine veränderte Interaktion mit der NADPH:P450-Oxidoreduktase oder Cytochrom b5 verantwortlich gemacht werden. Für das Zytostatikum Thiotepa und die Thrombozytenaggregationshemmer Clopidogrel und Ticlopidin wurde eine irreversible und potente Inhibition von CYP2B6 nachgewiesen, welche durch Hemmung der CYP2B6- Aktivität zu klinisch relevanten Arzneimittelinteraktionen mit Substraten dieses Enzyms führen kann. Als weitere signifikante Einflussfaktoren für die Expression und Funktion von CYP2B6 stellten sich das Alter und der Konsum bestimmter Arzneimittel dar. Das Alter der Patienten, von denen die Leberproben stammten, war umgekehrt proportional zur Apoproteinexpression und Bupropionhydroxylaseaktivität. Die präoperative Einnahme des Antiepileptikums und bekannten CYP2B6- Induktors Carbamazepin oder des Analgetikums Metamizol führte zu einer signifikant erhöhten Proteinexpression und Bupropionhydroxylaseaktivität bei diesen Patienten. An humanen Hepatozyten konnte eine Induktion von CYP2B6 durch diese Arzneistoffe nachgewiesen werden. Die Einnahme des Vasodilatators ISDN verursachte dagegen eine signifikante Reduzierung der Expression und Funktion von CYP2B6. Andere Faktoren wie Geschlecht, Alkohol- und Tabakkonsum hatten keinen Einfluss. Zusammenfassend konnte gezeigt werden, dass CYP2B6 von genetischen, biologischen und Umwelteinflüssen reguliert wird. Diese Ergebnisse können für eine verbesserte Vorhersage der CYP2B6 Enzymaktivität beim Menschen genutzt werden.

Translation of abstract (English)

Cytochrome P450 enzymes are the most important drug metabolizing enzymes in human liver. Their expression and function is regulated by biological, genetic and environmental factors. In this work, the influence of these parameters was analyzed for the poorly investigated cytochrome P450 enzyme CYP2B6. The expression and function was investigated in a human liver bank consisting of more than 100 samples. The CYP2B6 protein expression, which was quantified by immunoblot analysis, varied 191 fold (0.5 – 95.5 pmol/mg). For functional investigations two postulated model substrates for CY2B6 were chosen and compared to each other. The results demonstrated that bupropion is a more specific substrate than S-mephenytoin. Different mutations in the CYP2B6 gene were identified as one reason for the variable expression and function. In human liver samples and in recombinantly expressed CYP2B6 variants, the most frequent alleles *5, *6 and *15 could be associated with a significantly reduced expression and function compared to the wild- type (*1/*1). Some further and rare occuring mutations in the CYP2B6 gene even resulted in the absence of CYP2B6- expression and function. For some mutations it was shown that the reason for reduced enzyme acitivity is not only reduced protein expression but altered interaction with NADPH:P450- oxidoreductase or cytochrome b5. An irreversible and potent inhibition of CYP2B6 was shown for the cytostatic agent thioTEPA and the antiplatelet compounds clopidogrel and ticlopidine, which can result in clinically relevant drug interactions with substrates of CYP2B6 by inhibition. Further significant factors influencing expression and function were age and the consumption of drugs. The age of the liver donors was inversely proportional to the expression of CYP2B6- apoprotein and bupropion hydroxylase activity. The intake of the antiepileptic drug and known inducer of CYP2B6, carbamazepine, before surgery resulted in significantly increased expression and function of CYP2B6. The analgetic metamizole was newly identified as a potent inducer of CYP2B6. Induction of CYP2B6 could be demonstrated using human hepatocytes. In contrast, taking the antianginal drug ISDN caused a significantly declined expression and function. Some other factors like gender and the consumption of alcohol and tabacco had no influence. In summary, the results demonstrated, that CYP2B6 is regulated by genetic, biological and environmental factors. These results will allow a better prediction of CYP2B6 function in humans.
