Abstract
Bewegungstherapeutische Maßnahmen gelten bei der Behandlung der Adipositas als unerlässlich und haben sich im klinischen wie im ambulanten Bereich im Rahmen multimodaler Therapiekonzepte als feste Bestandteile durchgesetzt. In dieser bewegungswissenschaftlichen Arbeit wird zunächst das Krankheits- und Erscheinungsbild der Adipositas beschrieben. Die besondere Bedeutung der körperlich-sportlichen Aktivität und des Körperbildes wird in ihrer Relevanz für die Behandlung von Adipositas dargestellt. Es wird über die gängigen Behandlungsansätze im Sinne eines multimodalen Therapiekonzeptes berichtet, wobei die Verhaltenstherapie mit ihren Verfahren hier eine besondere Gewichtung erfährt. Weiterhin wird ein ausführlicher und aktueller Überblick über den Stand der bewegungstherapeutischen Interventionen bei Adipositas gegeben. Auch in diesem Abschnitt werden insbesondere die psychologischen Gesichtpunkte der sportlichen Aktivitäten und ihre mögliche Verknüpfung mit verhaltenstherapeutischen Verfahren in den Vordergrund gestellt. Kernstück der Arbeit ist die wissenschaftliche Überprüfung der Effekte der Bewegungstherapie. Das „Ambulante psychoedukative Bewegungsprogramm“ wird in einem kontrollierten Zweigruppen-Vergleich (N=58) auf seine Wirksamkeit hin überprüft. Durch den Vergleich mit einem Fitnesstrainingsprogramm können insbesondere die psychischen Effekte von Bewegungstherapie bei Adipositas aufgezeigt werden. Die Untersuchungsergebnisse werden umfassend dargestellt und diskutiert. Es ergeben sich Hinweise auf die therapeutische Relevanz des psychoedukativen Bewegungsprogramms für eine langfristige erfolgreiche Adipositastherapie.
Translation of abstract (English)
Movement therapeutic programs are irreplaceable in the treatment of obesity and a well established component of inpatient and outpatient multidisciplinary intervention concepts. This thesis describes the pathology and phemenology of obesity. The particular meaning of physical activity and body image is presented in its relevance for the treatment of obesity. Common treatment approaches such as multimodal therapy concepts are discussed, with a special focus on behavior therapy. The study furthermore gives a current overview of the state of art of movement therapeutic interventions in obesity and emphasizes the psychological aspects of physical activity and its connection to behavior therapy. The scientific evaluation of the effects of movement therapy constitutes the core of the study and compares two groups of n=30 and n=28 in a controlled design. The main intervention, an outpatient psychoeducational movement program, highlights the psychological effects of movement therapy for obesity and is tested against a fitness program. The results, which are presented and discussed in detail, suggest that the psychoeducational movement program is therapeutically relevant for a successful long-term therapy of obesity.
