Abstract

Angesichts des demographischen Wandels nimmt der Anteil älterer Menschen in der Bevölkerung stetig zu. In diesem Zusammenhang gewinnt das Problem degenerativer und arthrotischer Gelenkveränderungen zunehmend an Bedeutung. Dies zeigt sich unter anderem an der stetig zunehmenden Anzahl von Implantationen von Hüft- und Kniegelenkendoprothesen. So ist die Anzahl der Implantationen eines künstlichen Gelenkersatzes in den vergangenen Jahren konstant angestiegen. Deutschlandweit beträgt die Anzahl der Implantationen am Kniegelenk bereits 119.000, am Hüftgelenk bereits 180.000 pro Jahr. Während früher dieser Eingriff eher an älteren Patienten mit dem Ziel der Beschwerdefreiheit durchgeführt wurde, wird der Eingriff mittlerweile auch immer häufiger an jüngeren Patienten durchgeführt. Durch die immer weiter ausgereiften Operationstechniken steht zunehmend der Wunsch von körperlicher und sportlicher Betätigung nach dem Eingriff im Vordergrund. Darüber, in wieweit eine sportliche Aktivität nach Implantation eines künstlichen Gelenkersatzes möglich ist, liegen noch nicht ausreichend wissenschaftlich belegte Daten vor. In der vorliegenden Arbeit wird das sportliche Verhalten von Hüft- und Knieendoprothesen präoperativ und drei Jahre postoperativ untersucht. Die sportliche Aktivität von Endoprothesenträgern präoperativ und postoperativ ist ein bislang wissenschaftlich nur wenig untersuchtes Themengebiet. Es handelt sich in dieser Arbeit um eine Beobachtungsstudie zweier Kollektive (präoperativ und postoperativ). In dieser Arbeit wird zunächst das Krankheitsbild der Hüft- und Kniegelenkarthrose beschrieben. Es wird über die gängigen Behandlungsmöglichkeiten berichtet. Hierbei erfährt der endoprothetische Gelenkersatz inhaltlich die meiste Gewichtung. Weiterführend wird die Rehabilitation mit Schwerpunkt auf der Sporttherapie dargestellt. Weiterhin wird anhand einer ausführlichen Literaturübersicht nationaler sowie internationaler Studien ein aktueller Stand der Forschung bezüglich des Sports mit Hüft- bzw. Knieendoprothese vorgestellt. Die Untersuchung wird anhand zweier unverbundener Stichproben durchgeführt. Präoperativ beträgt das Patientenkollektiv N=967, postoperativ N=600. Präoperativ erfolgt die Befragung anhand eines halbstandardisierten Leitfadeninterviews, postoperativ erfolgt die Befragung postalisch. Der Untersuchung werden vier Hypothesen zu Grunde gelegt. Dies sind die Folgenden: a) Patienten sind postoperativ sportlich aktiver als präoperativ, b) Patienten sind postoperativ körperlich aktiver als präoperativ, c) Träger einer Hüftendoprothese sind sportlich aktiver als Träger einer Knieendoprothese und d) Patienten mit niedrigerem Body-Mass-Index sind sportlich aktiver als Patienten mit höherem Body-Mass-Index. Die Ergebnisse werden ausführlich dargestellt. Es folgt eine Zusammenfassung und Interpretation ausgewählter Ergebnisse. Diese werden mit dem aktuellen Stand der Forschung in Beziehung gesetzt. In dieser Untersuchung wird bestätigt, dass Patienten mit einem künstlichen Hüft- oder Kniegelenk postoperativ sportlich aktiver sind als präoperativ.

Translation of abstract (English)

In view of the demographic change the population rate of elderly people increases constantly. In this context, the problem of degenerative and arthritic changes of the joints has become of increasing importance for clinical research. Among others, the physical, degenerative changes become apparent in the constantly increasing quantity of implantations of hip joint and knee joint endoprostheses. Therefore the quantity of implantations of an artificial joint has been increasing constantly during the past decades. In Germany, the annual implantation rate of artificial knee joints is 119.000 and 180.000 of artificial hip joints. While in the past, this surgical procedure was mainly done on elderly people to alleviate pain and discomfort, nowadays this surgical procedure is also performed frequently on younger patients. The desire for sportive activities after surgery has risen with the improvement of implants and surgical procedures. So far it is not enough scientifically proven as to what extent sportive activities are possible after a total joint replacement. In the present thesis, a comparison is made between the sportive behavior of patients with hip joint and knee joint endoprostheses preoperative and three years postoperative. The sportive activities of patients with implanted endoprostheses preoperative and postoperative is a topic, which so far has not been studied scientifically in depth. The present thesis involves an observation study with two collectives (preoperative and postoperative). In the first part of the thesis, the disease pattern of the hip joint and knee joint osteoarthritis is characterized and the usual therapeutic treatment options are described. Here, special emphasis is directed to the total joint replacement and Sports Therapy as means of rehabilitation. Moreover, a detailed review of national and international literature outlines the current state of research concerning the sportive activities with hip joint and knee joint endoprostheses. The study was conducted with two unrelated samples. A total of N=967 preoperative and N=600 postoperative patients were evaluated in this study. The survey of preoperative patients was accomplished with the aid of semi-standardized interviews and the survey of postoperative patients was accomplished postally. Four hypotheses were taken as a basis for this study. These are the following: a) Patients do more sportive activities postoperative than preoperative b) Patients are postoperative more physically active than preoperative c) Patients with an implanted hip joint endoprosthesis do more sportive activities than patients with an implanted knee joint endoprosthesis and d) Patients with a lower Body-Mass-Index do more sportive activities than patients with a higher Body-Mass-Index. The results are being presented in full detail. A summary and an interpretation of selected results follows. These results are being correlated with the current state of research. In this thesis the hypothesis is being proved that patients with an artificial hip joint or knee joint do postoperative more sportive activities than preoperative.
