Abstract

Die Bioverfügbarkeit oral verabreichter Arzneistoffe wird vor allem durch das Ausmaß der Absorption durch das Epithel der Darmschleimhaut bestimmt und kann durch aktive Transportprozesse erheblich beeinflusst werden. So können Efflux-Transporter die intestinale Absorption stark herabsetzen, in dem sie in Enterozyten aufgenommene Arzneistoffe aus den Zellen zurück in das Darmlumen transportieren und Aufnahme-Transporter die Absorption von nicht zellgängigen Arzneistoffen erhöhen, in dem sie diese aus dem Darmlumen in Enterozyten aufnehmen. Kenntnisse über das Expressionsmuster, das Substratspektrum und die Aktivität dieser Arzneistoff transportierenden Transporter sind daher von großer Bedeutung für die Entwicklung neuer oraler Arzneimittel, die in vielen Fällen Substrate dieser intestinalen Transporter sind. Im Rahmen der hier vorliegenden Arbeit wurde zunächst die Expression der wichtigsten in Enterozyten vorkommenden Efflux-Transporter -Pgp, Bcrp und Mrp2- und der Aufnahme-Transporter Oatp1a5 und Oatp2b1 entlang des Darms von Ratten analysiert. Anschließend wurde exemplarisch für Pgp die in vivo-Relevanz des detektierten Expressionsmusters funktionell durch in vivo-Absorptionsstudien in Bereichen des Darms mit unterschiedlicher Pgp-Expression überprüft. Parallel wurden isolierte Ratten-Enterozyten als zelluläres Modellsystem für Transporterstudien charakterisiert. Die gewonnenen Daten wurden mit Transportstudien in Caco-2-Zellen, einem etabliertem Modell der intestinalen Barriere, verglichen. Die durchgeführten Expressionsanalysen zeigten für die untersuchten Transporter spezifische Expressionsmuster entlang des Darms, die auf mRNA- und Protein-Ebene vergleichbar waren. Die Expression von Pgp und Oatp2b1 nahm entlang des Darms kontinuierlich zu während für Bcrp und Oatp1a5 ein bogenförmiger Expressionsverlauf mit einer Zunahme entlang des Dünndarms und einer folgenden Abnahme im Dickdarm detektiert werden konnte. Die Expression von Mrp2 nahm dagegen entlang des Darms kontinuierlich ab. Für alle Transporter konnte eine erhebliche inter- und intraindividuelle Variabilität der Expression festgestellt werden. Geschlechtsspezifische Unterschiede der Expression zeigten sich vor allem im vorderen Bereich des Dünndarms. Die durchgeführten in vivo-Absorptionsstudien zeigten, dass die Expressionsstärke von Pgp mit dem Maß der intestinalen Absorption korrelierte. In Bereichen hoher Pgp-Expression wurde wenig Substrat absorbiert, in Bereichen niedriger Pgp-Expression viel Substrat. Im Rahmen der erfolgten zellulären Transportstudien gelang es, viable und transportaktive Enterozyten aus der Ratte zu isolieren. Die Zellen zeigten eine zeit- und konzentration-sabhängige Aufnahme verschiedener Transportersubstrate, die spezifisch gehemmt werden konnte. So konnte eine Oatp-vermittelte Substrat-Aufnahme, die Aktivität des Peptid-Transporters PepT1 und die aktive Aufnahme neutraler Aminosäuren durch verschiedene Aminosäure-Transporter in die Enterozyten nachgewiesen werden. Efflux-Transporter scheinen in den Zellen hingegen nicht aktiv zu sein. Damit stehen die Ergebnisse im Gegensatz zu den in Caco-2-Zellen erfolgten Transportstudien, die eine deutliche Pgp-Aktivität der Zellen zeigen konnten. Zusammengenommen zeigen die Ergebnisse, dass isolierte Ratten-Enterozyten verschiedene Aufnahme-Transporter exprimieren, die relevant für eine aktive intestinale Absorption von Arzneistoffen sein können. Daher stellt dieses primäre Zell-Modell ein geeignetes Werkzeug zur Bewertung des Absorptionsverhaltens von Arzneimittelkandiaten dar und ergänzt das Caco-2-Modell, das in erster Linie zur Identifizierung von Effux-Interaktionen genutzt wird.

Translation of abstract (English)

The bioavailability of orally administered drugs is mainly dominated by the amount that is absorbed across the intestinal epithelium and can be limited by active transport processes. Efflux transporters can decrease the intestinal absorption of drugs by transporting them out of the enterocytes back to the gut lumen, while uptake transporters can increase the absorption of impermeable compounds by an active uptake from the gut lumen into the enterocytes. Knowledge about the pattern of expression, substrate spectrum, and activity of intestinal drug transporters is therefore crucial for the development of new orally administered drugs, which are in many cases substrates of drug transporters. Within the framework of the presented thesis, first of all, the pattern of expression of the most important intestinal efflux transporters –Pgp, Bcrp and Mrp2- and of the uptake transporters Oatp1a5 and Oatp2b1 was analyzed along the intestinal tract of rats. To assess the in vivo relevance of the detected patterns of expression, the in vivo absorption of drugs was studied exemplarily for Pgp in parts of the intestine with different levels of Pgp expression. In addition, isolated rat enterocytes were characterized as a cellular model of drug transport. The obtained data were compared to transport studies performed in Caco-2 cells, which are an established cell-based model of the intestinal barrier. The performed analysis of expression revealed, that specific patterns of expression exist for the transporters being investigated. Pgp and Oatp2b1 increased along the intestinal tract, whereas Bcrp and Oatp1a5 showed an arcuate pattern of expression with an increase along the small intestine followed by a decrease in the large intestine. Mrp2 decreased continuously along the intestine. A pronounced inter- and intraindividual variability of expression could be detected for all transporters being investigated. Gender-specific differences of transporter expression could be mainly found in the upper small intestine. The in vivo study of drug absorption showed, that the level of Pgp expression correlated well with the amount of Pgp substrate absorbed across the intestinal epithelium. The higher the Pgp expression, the lower the intestinal absorption of the Pgp substrate was and vice versa. In order to establish a cellular model to study intestinal drug transport, viable rat enterocytes could be isolated which were suitable for transport studies. The isolated cells showed a time- and concentration-dependent uptake of substrates of various transporter families which could be selectively inhibited. An Oatp-mediated uptake of substrates, as well as the activity of the peptide transporter PepT1 and the active uptake of neutral amino acids by amino acid transporters into the isolated enterocytes could be confirmed. Efflux transporters however, seemed not to be active in isolated enterocytes. These data are in contrast to the transport studies performed with Caco-2- cells, which showed a pronounced activity of Pgp. Taken together, these data indicate that isolated primary rat enterocytes express several uptake transporters which may be relevant for the active intestinal absorption of drugs. Therefore, this primary cell model may be a suitable tool for evaluation absorption behavior of drug discovery compounds in addition to the Caco-2-model, which is mainly used as a model of drug efflux.
