Abstract

Der Darm ist kein homogen aufgebautes Organ, sondern unterliegt in der Morphologie, als auch in seiner Proteinverteilung Veränderungen vom proximalen bis distalen Ende. Da der intestinale Metabolismus eine große Auswirkung auf die Bioverfügbarkeit von oral aufgenommen Arzneistoffen besitzen kann, basiert eine erfolgreiche Arzneimittelentwicklung auch auf dem umfangreichen Wissen über metabolische Enzyme entlang des Darms. Wie die Hepatozyten, enthalten ebenso die Enterozyten ein Set an verschiedenen Cytochrome P450 (CYP P450) und anderen metabolisierenden Enzymen der Phase-I und -II. Obwohl die involvierten Enzyme für die Leber gut charakterisiert sind, existieren nur wenige Informationen über ihre Expression/Funktion in der Darmwand und entlang des gesamten Darms. Im Rahmen dieser Doktorarbeit konnte gezeigt werden, das mehrere CYP P450 Enzyme (CYP3A, 2B1, 2C6, 2D1, 1A1) und weitere Enzyme der Phase-I/-II (17ß-HSD2, UGT) im Darm exprimiert werden. Außerdem wurde gezeigt, dass CYP2B1 und 3A eine kontinuierlich verminderte Expression entlang des Darms aufweisen, und dominant in den Zellen der Zottenspitze exprimiert werden, also genau dort wo die meisten Nähr- und Arzneistoffe absorbiert werden. Mengenmäßig stellen CYP2B1 (ca. 65 %) und 3A (ca. 28 %) die dominanten CYP P450 Isoformen im proximalen Darm der Ratte dar. Dagegen spielen die Enzyme CYP2C6 und 2D1 im Darm eine eher untergeordnete Rolle. Daraus kann die Hypothese abgeleitet werden, dass diese beiden Enzyme eher eine Rolle im endogenen Abbauprozess spielen, als im Metabolismus von Arzneistoffen. Im Rahmen dieser Doktorarbeit konnte weiterhin gezeigt werden, dass der proximale Darm im Vergleich zur Leber circa 15fach niedrigere und der distale Darm circa 100fach niedrigere CYP P450 Mengen enthält. Auch ist die CYP P450 Verteilung in der Leber ganz anders, als im Darm. Während im Darm CYP3A und 2B1 dominieren, stellen in der Leber alle untersuchten CYP P450 Enzyme gleichgroße Fraktionen an der CYP P450 Gesamtsumme. Des Weiteren wurden zwei unterschiedliche in vitro Modelle eingesetzt um den intestinalen Metabolismus zu charakterisieren. Zum einem wurden isolierte primäre Enterozyten der Ratte verwendet, zum anderen intakte Darmsegmente der Ratte („Everted Sacs“). Beide Modelle lieferten vergleichbare Abbauraten der untersuchten Test-Substanzen: Dextromethorphan, Testosteron, Midazolam und Bupropion. Basierend auf den Abbauraten konnte die intrinsische Clearances (CLint) Werte für alle Test-Substanzen in beiden Modellen errechnet werden. Von allen untersuchten Test-Substanzen zeigt Testosteron den höchsten Abbau im Darm. Dieser Abbau erfolgt dabei hauptsächlich über das Phase-I-Enzym 17ß-HSD2. Die anderen Test-Substanzen werden dagegen hauptsächlich über CYP P450 Enzyme abgebaut, was auch über Versuchsansätze mit Aminobenzotriazol (ABT), einem irreversiblen CYP P450 Inhibitor, bestätigt werden konnte. Des Weiteren konnte anhand der CLint ES-Werte der Testsubstanzen und unter Zuhilfenahme von ermittelten Skalierungsfaktoren die CLint DR der Test-Substanzen errechnet werden. Aus diesen Ergebnissen konnten wiederum mittels „QGut Modell“ die „First pass“ in vivo Clearances (CLFP Darm) und der Substanzanteil der unmetabolisiert die Darmwand übertritt (FG-Werte) für alle Test-Substanzen berechnet werden. Dabei errechnete sich für Testosteron (4.7 mL/min/kg Körpergewicht) und Bupropion (3.4 mL/min/kg Körpergewicht) die höchsten CLFP Darm-Werte und die niedrigsten FG-Werte für die proximalen Darmregionen. Im distalen Dünndarm sinkt für beide Substrate der Metabolismus stark ab, was eine Erhöhung der FG-Werte zur Folge hat. Neben den „Everted Sacs“ können auch primäre Enterozyten für die Vorhersage von intestinalen Metabolismus und Absorption eingesetzt werden. Allerdings erfordert eine valide Vorhersage eine reproduzierbare hohe Vitalität der Enterozytenkulturen. Die Methode wurde im Rahmen dieser Doktorarbeit aufgebaut, aber noch weitere Schritte sind notwendig, um sie weiter zu optimieren und anwenden zu können.

Translation of abstract (English)

The intestine is not a homogenous designed organ with a changing morphology and changing protein expression profiles from its proximal to distal expansion. The intestine is capable of influencing the bioavailability of orally administered drugs. Therefore, a profound knowledge of metabolic enzymes along the intestine is necessary for a successful drug development. Alike hepatocytes, enterocytes contain a set of many different Cytochrome P450 (CYP P450) isoforms and other phase-I/-II metabolic enzymes. Although the metabolic enzymes of the liver are very well characterized, only few information exist about their expression and function in the rat gut wall and along the entire intestine. In this doctoral thesis the expression of several CYP P450 enzymes (CYP3A, 2B1, 2C6, 2D1, 1A1) and other phase-I/-II-enzymes (17ß-HSD2, UGT) along the entire intestine was shown in spatial detail. CYP2B1 and CYP3A show decreasing expression profiles along the rat intestine and are not or very low expressed in the large intestine. Both enzymes are dominantly expressed in the very upper tip of the villi of the proximal intestine and therefore in the nutrient and drug dominant absorption area. Furthermore, CYP2B1 (65 %) and 3A (ca. 28 %) are the predominant CYP P450 isoforms in the proximal intestine, while CYP2C6 and 2D1 just play minor roles there. This may suggest that CYP2C6 and 2D1 are more likely to be involved in the metabolism of endogenous, rather than exogenous substrates. In comparison with the liver, the proximal intestine of the rat contains 15 times and the distal intestine 100 times lower CYP P450 amounts. Furthermore, the liver shows a completely different CYP P450 distribution. While in the rat intestine CYP3A and CYP2B1 dominate, all examined CYP P450 isoforms contribute similar fractions to the whole CYP P450 amount in the liver of the rat. Furthermore, two in vitro models were established to characterize the intestinal metabolism. Primary enterocytes of the rat intestine and intact gut segments of the rat (“Everted Sacs”) were prepared for the functional assays. Both in vitro models led to comparable metabolic degradation rates for all test compounds investigated: testosterone, midazolam, dextromethorphan and bupropion. For all compounds the intrinsic clearance (CLint) was calculated. Testosterone showed the highest intestinal metabolism, which is mainly the consequence of 17ß-HSD2 activity. The other compounds are predominately metabolized by CYP P450 isoforms, as incubations with the CYP P450 inhibitor, aminobenzotriazole (ABT), showed. Based on the CLint ES values and specific scaling factors, the intrinsic clearance for all test-compounds was calculated for the respective intestinal regions (CLint DR). By using the “QGut model” it was possible to calculate the first pass in vivo clearance (CLFP DR) and the unchanged absorbed fraction of the test-compound (FG). Of all compounds investigated, testosterone and bupropion showed the highest first pass in vivo clearances and the lowest FG values in the proximal intestine. Along the intestine, the metabolism of both compounds decreased strongly and therefore, an increase in the FG values could be observed. Besides the “Everted Sac” model also primary enterocytes could be used for the prediction of intestinal metabolism and absorption. The base for a valid prediction is a reproducible vital cell culture. The isolation of vital primary enterocytes needs further investigation.
