Abstract

Unter der Bezeichnung „Epigenetik“ fasst man diejenigen Mechanismen zusammen, die es Zellen ermöglichen entsprechend ihrer Funktion und Umgebung ein individuelles Genexpressionsmuster zu etablieren und ohne Veränderung der DNA-Sequenz zu vererben. Die individuellen Informationen werden in der Zelle durch chemische Veränderungen der Chromatinbestandteile, wie die posttranslationalen Modifikationen an Histon-Proteinen oder die Methylierung der DNA, definiert und beeinflussen die Aktivität der betroffenen Abschnitte des Genoms. Störungen dieser Regulationsmechanismen können zur Aktivierung von Onkogenen und zur Inaktivierung von Tumorsuppressorgenen führen und die maligne Entartung von Zellen verursachen. Im Gegensatz zu genetischen Mutationen sind die auf biochemischen Modifikationen des Chromatins beruhenden Epimutationen reversibel. Diese Eigenschaft wird von epigenetischen Therapieansätzen genutzt, deren Ziel es ist, durch die Inhibition bestimmter Enzyme das Ungleichgewicht in der epigenetischen Regulation zu beheben. Im ersten Teil der vorliegenden Arbeit konnte gezeigt werden, dass der Histon-Deacetylase (HDAC)-Inhibitor Valproat (VPA) die Apoptoserate von ex vivo kultivierten CLL-Patienten-Zellen erhöht. Es stellte sich heraus, dass durch den Einfluss des HDAC-Inhibitors die Sensitivität der Zellen für die Induktion der Apoptose wieder hergestellt wurde. Dies geschah über die Aktivierung der klassischen Apoptosewege, wie durch die Aktivierung von Caspase 8 und 9 sowie die Beteiligung verschiedener Proteine der Bcl-2-Familie nachgewiesen werden konnte. Die Untersuchungen ergaben außerdem, dass CLL-Zellen deutlich empfindlicher auf VPA reagieren, als Zellen von gesunden Probanden. Somit ergaben sich neue Erkenntnisse über die Wirkungsweise dieses HDAC-Inhibitors in CLL-Zellen, die VPA allein oder in Kombination mit anderen Substanzen als eine Option für die Therapie von CLL-Patienten darstellen. Im zweiten Teil dieser Arbeit wurde gezeigt, dass die Behandlung mit dem DNA-Methyltransferase-Inhibitor 5-Azacytidin bei MDS-Patienten zu einer reversiblen DNA-Hypomethylierung führte, die sowohl für das gesamte Genom als auch für spezifische Sequenzen nachweisbar war. Das klinische Ansprechen der Patienten war nicht mit der festgestellten Demethylierung korreliert. Ebenso ergab sich bei den untersuchten Genen nur teilweise ein Zusammenhang zwischen den Veränderungen der Genexpression und der Promotor-Methylierung. Somit konnte die Wirksamkeit von 5-Azacytidin bei der Reaktivierung hypermethylierter Gene festgestellt werden, um die Bedeutung dieses Mechanismus für Therapieerfolge aufzuklären, sind jedoch weitere Untersuchungen notwendig.

Translation of abstract (English)

Epigenetics comprises mechanisms that initiate and maintain heritable patterns of gene expression according to cell function and microenvironment without changing the sequence of the genome. The “epigenetic code” is established by chromatin modifying enzymes like Histone deacetylases (HDACs) or DNA methyltransferases (DNMTs) and determines the activity state of affected genomic regions. Aberrant histone acetylation or DNA methylation may promote the activation of oncogenes or the silencing of tumor suppressor genes. These so called “epimutations”, play an important role in the pathogenesis of cancer. In contrast to genomic mutations, epimutations are reversible. This property is used for epigenetic treatment strategies which aims for the compensation of the imbalanced enzyme activities. In the first part of this work, blood cells of patients with Chronic Lymphocytic Leukemia (CLL) were treated with valproic acid (VPA), a well-established histone deacetylase (HDAC) inhibitor. The main feature of the disease is the prolonged survival of B-cells that is being caused by aberrant apoptosis. It was observed that VPA mediates apoptosis in CLL cells ex vivo through caspase activation via both, the extrinsic and the intrinsic apoptosis pathways, as indicated by the activation of the caspase proteins 8 and 9, and cleavage of the proapoptotic protein BID. The Bcl-2/Bax ratio was decreased as a consequence of decreased bcl-2 mRNA levels in response to treatment with VPA. With the results presented in this study, we have identified the HDAC inhibitor VPA to restore the apoptotic pathways in CLL cells and thus their ability to undergo apoptosis. The second part of this work shows the results of a study on Myelodysplastic Syndrome (MDS) patients treated with the DNMT inhibitor 5-azacytidine. A number of clinical trials with 5-azacytidine have reported durable improvements in bone marrow function and a reduction in the risk of leukemic transformation in MDS patients when compared to best sup¬portive care. However, the demethylation characteristics of the drug have not been determined yet. The treatment of 10 patients was monitored and clinical and hematological parameters, as well as DNA methylation and gene expression markers were analyzed at weekly intervals. Significant clinical and hematological improvements, particularly in high-risk MDS patients, were observed. Also transient global DNA demethylation was detected in a subset of patients. However, the degree of 5-azacytidine mediated demethylation did not clearly cor¬relate with the hematological response or epigenetic alterations at specific genomic loci. The study presents a detailed analysis of 5-azacytidine-induced epigenetic changes in MDS patients and suggests that the mode of action does not strictly depend on the demethy¬lating activity of the drug.
