Abstract
Trotz intensiver Forschungsbemühungen ist es bis heute nicht gelungen, die Ätiologie des Phantomschmerzes aufzuklären und effektive Behandlungsmöglichkeiten anzubieten. Im ersten Teil der Arbeit wird die prothetische Versorgung sowie die Evaluation eines strukturierten Interviews zur Erfassung des Prothesengebrauchs nach Amputation der oberen Extremität vorgestellt. Angaben über die Funktionsübernahme des gesunden Armes, sowie über weitere psychophysiologische Phänomene des Prothesengebrauchs lassen sich ebenfalls mit dem Interview erheben. Im zweiten Teil der Arbeit wird der Zusammenhang von Prothesengebrauch und Phantomschmerzen untersucht. Hier zeigt sich vor allem ein spezifischer, negativer Zusammenhang zwischen dem Ausmaß des Gebrauchs einer myoelektrisch gesteuerten Prothese und dem Ausmaß des Phantomschmerzes. Der dritte Teil der Arbeit befasst sich mit einem medikamentösen Behandlungsansatz des Phantomschmerzes. Durch die Gabe des NMDA-Rezeptor-Antagonisten, Memantine, konnte in einer kontrollierten Behandlungsstudie im Doppel-blind-crossover-design der Phantomschmerz gegenüber der Placebobehandlung signifikant verringert werden.
Translation of abstract (English)
In spite of extensive research, the etiology of phantom limb pain is currently not completely understood and there is still a lack of effective treatments. The first part of the present work gives an introduction to the prosthetic management of upper limb amputations and presents the psychometric evaluation of a structured interview on prosthesis usage after upper limb amputation. The interview also includes information on the compensatory function of the intact arm and on further psychophysiologic phenomena associated to prosthesis usage. In the second part, the relation between prosthesis usage and phantom limb pain is investigated. The main result is a specific negative association between the extent of usage of a myoelectric upper limb prosthesis and the intensity of phantom limb pain. The third part focuses on a pharmaceutical treatment approach for phantom limb pain. It is shown that the administration of a NMDA receptor antagonist, Memantine, in a controlled trial using a double blind - crossover design reduced phantom limb pain significantly compared with a placebo treatment.
