Abstract

Thema der vorliegenden Arbeit ist die Anwendung einer neuartigen Methode der Glaukombekämpfung mittels Laserstrahlung. Unter Verwendung einer Spaltlampe, die zur Laserstrahleinkopplung und zur Beobachtung dient, wird der Laser über ein Kontaktglas von außen in den Kammerwinkel eingekoppelt, das Auge muß also nicht geöffnet werden. Durch dieses non-invasive Verfahren erhofft man sich eine wesentlich geringere postoperative Vernarbung. Zum Einsatz kommen drei unterschiedliche Kurzpuls-Lasersysteme; zwei Pikosekundenlaser sowie ein Femtosekundenlaser. Aufgrund der kurzen Pulsdauer findet keine thermische Erhitzung des Gewebes statt, so daß der Ablationsprozeß in allen Fällen sehr schonend verläuft. Für die Versuche wurden frische enukleierte Schweineaugen verwendet. Sowohl mit dem Pikosekundenlaser als auch dem Femtosekundenlaser konnten Ablationen des Trabekelwerks erzielt werden. Dabei erweist sich jedoch das korrekte Einkoppeln des Laserstrahls in den Kammerwinkel als recht schwierig. Neben den Experimenten mit dem Kontaktglas befaßt sich die Arbeit mit der Bestimmung von Ablationsschwellwerten für das Trabekelwerk. Anhand von Elektronenmikroskopaufnahmen konnten Ablationskurven ermittelt werden. Die aus diesen Kurven erzielten Schwellen von ca. 16,5 J/cm², 13 J/cm² (ps-Laser) bzw. 2,3 J/cm² (fs-Laser) bestätigen die theoretische Abhängigkeit der Ablationsschwelle von der Pulsdauer.

Translation of abstract (English)

The theme of this thesis is the application of a new method in the treatment of glaucoma using laser radiation. By the use of a slitlamp which serves for the incoupling of the laser beam and for the monitoring, the laser is coupled from the outside into the eye by passing a contact glass. So, the eye does not have to be opened. This non invasive procedure tend to have a much smaller post operative scar formation. Three different short pulse laser systems were used for the experiments; two picosecond laser and one femtosecond laser. Due to the short pulse length no thermal heating of the adjacent tissue occurs, so the ablation process is very sparing in all cases. Fresh enucleated porcine eyes were used for the experiments. With both the picosecond laser and the femtosecond laser an ablation of the trabecular meshwork was possible. Thereby the correct incoupling of the laser beam into the chamber angle of the eye proved to be very difficult. Beside the experiments with the contact glass the thesis is about the determination of ablation threshholds for the trabecular meshwork. By means of electron microscopies ablation curves could be created. The threshholds of 16.5 J/cm², 13 J/cm² (ps-Laser) and 2.3 J/cm² (fs-laser) respectively which can be computered from the curves confirm the theoretical dependence of the ablation threshhold from the pulse duration.
