Development of a breathing phantom for comparison of two breathing surrogates systems in 4DCT-imaging

Abstract

Die vierdimensionale Computertomographie (4D-CT) benötigt ein Monitorsignal für die Atmung. Verschiede Systeme sind zur Zeit verfügbar, aber um diese Systeme unter realistischen Bedingungen zu testen, ist ein Phantom mit einem beweglichen Zielobjekt und einer verformbaren äußeren Kontur erforderlich. Ein anthropomorphes Phantom wurde entwickelt und getestet. Die Atmungsbewegung eines Probanden wurde zusammen mit einer synchronen und asynchronen Bewegungen eines Lungentumors reproduziert. Das Phantom besteht aus Gewebe-äquivalenten Materialen und ist daher auch für dosimetrische Studien einsetzbar. Mit Hilfe des Phantoms wurden ein optische Kamera-Systeme (GateCT, VisionRT Ltd.) und ein Atemgürtel mit einem Drucksensor (AZ-733V, Anzai Medical Co. Ltd) getestet und 4D-CTs (Somanton Sensation Open, Siemens AG) mit beiden Atmungssignalen rekonstruiert. Ein Vergleich beider rekonstruierten Tumortrajektorien zeigte signifikante Unterschiede. Diese konnten nicht durch sichtbare Unterschiede in den Atemkurven, wie zB. Baseline-drift, unterschiedliche Zeitauflösung oder größeres Rauschen, erklärt werden. Stattdessen wurde gezeigt, dass die unterschiedlichen Trajektorien von der Variabilität der Abtastintervalle und der Genauigkeit der Abtastfrequenz in der Datei der Atemkurve verursacht wurden. Wenn man die Atemkurve auf ein reguläres Abtastintervall interpoliert und die Abtastfrequenz auf einen genauen Wert setzt, stimmen die rekonstruierten Trajektorien für beide Monitorsysteme überein. Ein Vergleich mit der vorgegebenen Trajektorie ergab für die verwendete CT-Schichtdicke von 3 mm eine residuale Unsicherheit von maximal 3 mm. Diese Unsicherheit muss bei der Definition des Zielvolumens berücksichtigt werden. Die Untersuchungsergebnisse heben die Wichtigkeit der Überprüfung neuer klinischen Geräte hervor, besonders wenn sie zusammen mit Geräten anderer Hersteller betrieben werden. Mit dem neu entwickelten Phantoms können klinische Workflows getested und die Funktionsfähigkeit der verwendeten medizinischen Geräte sichergestellt werden.

Translation of abstract (English)

Four-dimensional Computed Tomography (4D-CT) requires a surrogate breathing signal. Several surrogate systems are available, but for a realistic testing of these systems, a phantom with a moving target and a deformable outer contour is required. An anthropomorphic phantom was developed and tested to simulate patient breathing as well as the synchronous or asynchronous movement of a lung tumour. The phantom consists of tissue-equivalent materials and can be also used for dosimetric measurements. Using the phantom, an optical camera system (GateCT, VisionRT Ltd) and a belt with an integrated pressure sensor (AZ-733V, Anzai) were simultaneously operated and 4D-CTs (Somatom Sensation Open, Siemens AG) were reconstructed from both breathing curves. Comparison of the reconstructed tumour trajectories revealed significant differences. Visible differences in the breathing curves such as baseline shifts, a lower temporal resolution and an increased noise level were found in one of the signals, however, it was shown that these factors were not the reason for the observed differences. Subsequent experiments demonstrated that the differences were caused by the variability of the sampling interval and the accuracy of the sampling rate value in the file of the breathing curve. Interpolation to regular sampling intervals and correction of the sampling rate in the header to the actual value removed the differences encountered. For the used CT-slice thickness of 3 mm, a comparison with the predefined trajectory revealed a residual uncertainty of up to 3 mm. This uncertainty has to be considered in the definition of the target volume. The results of this study underline the importance of testing new equipment thoroughly, especially if components of different manufacturers are combined. Using the newly developed phantom, clinical workflows can be tested and the functioning of the applied medical devices can be ensured.
