Abstract

In der dynamischen suszeptibilitätsgewichteten Magnetresonanztomographie (MRT) wird die Signaländerung im Gewebe unter bolusförmiger intravenöser Applikation eines Kontrastmittels (KM) meßtechnisch erfasst. Misst man den charakteristischen Signal- Zeit-Verlauf nicht nur im Organgewebe, sondern gleichzeitig auch in einer zuführenden Arterie, so können mit Hilfe der Indikator-Verdünnungstheorie unter bestimmten Annahmen das regionale Blutvolumen (RBV), der regionale Blutfluss (RBF) und die mittlere Transitzeit (MTT) des KMs bildpunktweise für das Organgewebe berechnet werden. Die gemessenen Signal-Zeit-Kurven müssen hierzu in Konzentrations-Zeit-Verläufe konvertiert und die Gewebekurven anschließend mit der arteriellen Inputfunktion (AIF) entfaltet werden. Im Rahmen dieser Arbeit wurde eine Methodik entwickelt, um aus dynamischen MR-Datensätzen die Mikrozirkulation der Niere zu bestimmen. Besonderes Augenmerk lag bei der Auswertung der Datensätze auf der zur Gewinnung von quantitativen Parametern notwendigen Entfaltung. Es wurde ein Entfaltungsalgorithmus entwickelt, der auch für verrauschte Datensätze zu stabilen und reproduzierbaren Lösungen führt, was durch Simulationsrechnungen nach- gewiesen werden konnte. Im Rahmen einer tierexperimentellen Studie wurde die Technik validiert. Die autoregulatorische Kapazität der Niere, den regionalen Blutfluß auch bei starken arteriellen Blutdruckschwankungen aufrechtzuerhalten, konnte nichtinvasiv nachvollzogen werden. Im Rahmen von kleinen Patientenstudien erlaubte die entwickelte Methodik eine Differenzierung zwischen gesunden Nieren und solchen mit Schädigungen der Nierenrinde, die sich in verminderten Blutflusswerten manifestierten.

Translation of abstract (English)

In dynamic susceptibility-weighted magnetic resonance imaging (MRI) the signal variation during the passage of an intravenously administered contrast media (CM) is detected. Using principles of indicator dilution theory, hemodynamic tissue parameters - such as the regional blood volume (RBV), the regional blood flow (RBF), and the mean transit time (MTT) of the CM - can be determined from signal-time-curves evaluated in the considered tissue region as well as in an artery. To this end, the signal-time-curves must be converted to concentration-time-curves. Moreover, a deconvolution of the tissue curves with the arterial input function (AIF) is required. The aim of the work presented was to develop a method for the determination of microcirculatory parameters of the kidney from dynamic MRI data. Since deconvolution is a prerequisite for the estimation of absolute tissue values, the implementation of a reliable and fast deconvolution technique is of fundamental importance. Using Monte Carlo techniques it could be shown that the deconvolution algorithm presented yields reliable and stable solutions for noisy data sets. In an animal study the method was validated. The renal capacity of autoregularization, i. e. the capacity to maintain a constant regional blood flow over a wide range of arterial blood pressure, was reproduced non-invasively. In a limited number of patients it could be shown that a differentiation between healthy and scarred kidneys was possible by means of the detected perfusion deficit.
