Abstract

Zur stereotaktischen und intensitätsmodulierten Strahlentherapie von Tumoren ist eine präzise Patientenpositionierung – aufgrund steilerer Gradienten der Dosisverteilung – essenziell. Die vorliegende Arbeit beschäftigt sich mit der Implementierung eines hochpräzisen, ?exiblen Stereokamerasystems zur Detektion von patientenfesten, kugelförmigen Markierungen zur videobasierten Patientenpositionierung. Hierzu berechnen wir 3D Positionen dieser Markierungen, die durch ein mathematisches Kameramodell – unter Berücksichtigung der radialen und tangentialen Linsenverzeichnung – beschrieben werden. Um die gewünschte Präzision zu erreichen, wurde ein neues Kalibrierphantom entwickelt und konstruiert. Es erlaubt eine geometrische Vor-Ort Kamerakalibrierung, was sehr vielseitige Anwendungen ermöglicht. Die Parameter, die im Kameramodell verwendet werden, stammen aus diesem Kalibrierverfahren. Zusätzlich erleichtert das Phantom eine regelmäßige Qualitätssicherung (QA) des Kamerasystems, da für den medizinischen Anwendungsfall die tägliche Einsatzfähigkeit gewährleistet und auch überprüfbar sein muss. Für den klinischen Einsatz des Stereokamerasystems werden zwei Anwendungen betrachtet. Einerseits steht die automatisierte Patientenpositionierung am Bestrahlungsgerät zur Diskussion. Andererseits wird eine vorbereitende Studie zur Echtzeitüberwachung der Patientenbewegung während der Bestrahlung präsentiert.

Translation of abstract (English)

In treating tumor patients with stereotactic and intensity modulated radiotherapy, precise patient positioning is essential due to the steep gradients of dose distribution. This work presents the implementation of an accurate and ?exible stereo camera system detecting patient-?xed spherical markers for video-based patient positioning. For this, we compute 3D marker coordinates in space, which are described by a mathematical camera model that considers radial and tangential lens distortion. To achieve the desired precision, a new calibration phantom was designed, developed, and constructed. It allows an on site geometrical camera calibration which enables a wide range of applications. The parameters used in the camera model are determined during this calibration procedure. In addition the phantom facilitates regular quality assurance (QA) routines of the camera system which is necessary to ensure daily operational integrity in clinical application. The use of the stereo camera system in clinical practice is considered for two applications. Firstly, automatic patient positioning in the treatment room is discussed, and secondly a preliminary study to monitor online movement of patients during irradiation is presented.
