Abstract

Es wurde eine Meßtechnik entwickelt und optimiert, mit der zeitlich und räumlich hochaufgelöste T1-gewichtete MR-Bilder zur dynamischen MRT aufgenommen werden, so daß sowohl eine hohe Ortsauflösung als auch eine Auswertung mit einem Zweikompartimentemodell ermöglicht wird. Die auf einer FLASH-Sequenz basierte Technik wurde mit einer etablierten zeitlich hochaufgelösten SR-TFL-Technik verglichen. Die Ortsauflösung der neuen Methode ist mit (1,5×1,5×4) mm³ höher als die mit der SR-TFL-Technik realisierte Auflösung von (3,0×2,0×6) mm³. Dagegen erreicht die FLASH-Technik nur eine Zeitauflösung von dt = 43 s gegenüber dt = 23 s der SR-TFL-Technik. Simulationen zeigen, daß trotz schlechterer zeitlicher Auflösung auch die mit der FLASH-Technik aufgenommenen Daten stabil an ein pharmakokinetisches Modell angepaßt werden können. Der Grund ist das deutlich höhere SNR der neuen Methode, welches in Phantomexperimenten fünfmal und in-vivo 3,3-mal höher war als bei der SR-TFL-Technik. Im Tiermodell wurde die Technik zur Untersuchung von Angiogenese eingesetzt. Im untersuchten Tumormodell bestand zwischen dem Modellparameter A und der Mikrogefäßdichte (MVD) ein Zusammenhang, während kep keine Korrelation mit der MVD aufwies. Ein Vergleich von Gadomer-17 und Omniscan® im Modell der Ratte zeigte, daß die FLASH-Technik in Kombination mit einem makromolekularen KM, die Untersuchung der Permeabilität von Tumorgefäßen ermöglicht. Schließlich wurde die neue Meßtechnik in ersten Patientinnenuntersuchungen zur Verbesserung der räumlichen Auflösung in der MR-Mammographie angewendet.

Translation of abstract (English)

A new technique was developped and optimized to measure T1-weighted MR images with both high spatial and temporal resolution in order to analyze dynamic MRI data with a two-compartment-model. The method which is based on a FLASH sequence was compared to an established SR-TFL sequence with high temporal resolution. (1,5×1,5×4) mm³ was the spatial resolution of the FLASH technique which was higher than (3,0×2,0×6) mm³ of the SR-TFL technique. In contrast, temporal resolution was lower with the FLASH method (dt = 43 s) compared to that of the established one (dt 23 s). Simulations showed that in spite of lower temporal resolution a stable analysis of the FLASH-data by using a pharmacokinetic model was assured. The reason was the higher SNR for which five-fold higher values in phantom experiments and 3,3-fold higher ones in-vivo were measured compared to the SR-TFL method. In animal models the new technique was used to investigate angiogenesis. In the examined tumor model a correlation of modelparameter A and microvessel-density (MVD) was found, whereas kep and MVD showed no correlation. A comparison of Gadomer-17 and Omniscan® demonstrated that permeability of tumor vessels can be measured by the FLASH technique in combination with a macromolecular contrast agent. Finally the new method was applied in the first patients in order to improve spatial resolution of MR mammography.
