Abstract

Die modernen Verfahren in der heutigen Strahlentherapie wurden dazu entwickelt, das Bestrahlungsfeld optimal auf den Tumor zu konzentrieren und dabei die applizierte Dosis im umgebenden gesunden Gewebe zu minimieren. Organbewegungen und Lagerungsfehler verhindern jedoch, dass mit diesen Techniken eine Präzision im Millimeterbereich erreicht werden kann. Daher setzt sich diese Arbeit zum Ziel, den Einfluss solcher Veränderung auf die Strahlentherapie zu analysieren und Strategien vorzustellen, die eine bessere Konformität der Bestrahlung gewährleisten. Für die interfraktionellen Variationen, also solche, die von einer Fraktion zu nächsten auftreten, wurden mit Hilfe von vier Patientendatensätzen aus mehreren CT-Aufnahmen Strategien zur täglichen Neuplanung, Lagerungskorrektur bzw. Korrektur eines systematischen Lagerungsfehlers untersucht, wobei eine signifikante Verbesserung der Dosisverteilung erreicht werden konnte. Die intrafraktionellen Variationen, wie z.B. atmungsbedingte Organbewegungen, wurden mit Hilfe eines bewegten Lungenphantoms analysiert. Hier müssen verschiedene Aspekte der Bildgebung und Bestrahlung zu einer vierdimensionalen Behandlungsstrategie vereint werden. Alle dafür benötigten Schritte wurden im Rahmen dieser Arbeit untersucht. Hierzu zählt im letzten Schritt auch eine atemgesteuerte Bestrahlungstechnik, mit der es möglich war, für Messungen mit dem dynamischen Lungenphantom eine mittlere Dosissteigerung im Zielvolumen von 90% auf etwa 99% der verschriebenen Dosis zu erreichen.

Translation of abstract (English)

The recent highly conformal radiation therapy techniques have been developed to restrict the radiation field to the tumor and to minimize the applied dose in the surrounding healthy tissue. However, often it is not possible to achieve an accuracy in the range of a few millimeters due to organ motions and setup errors. Therefore the aim of this work is to study the impact of these variations on the radiation therapy and to analyze strategies which allow for a better conformity of the irradiation. Interfractional variations of the anatomy, which occur in-between different fractions were analyzed using data sets of multiple CT scans. Different strategies like daily replanning, daily setup error correction or correction of a systematic error have been studied and a significant improvement of the dose distributions has been achieved. The intrafractional variations like respiratory organ motions were investigated using a moving lung phantom. In this field different aspects of image acquisition and radiation therapy have to be combined into a four-dimensional treatment strategy. The different steps required for such a treatment course have been studied in the thesis at hand. The last step in this chain is a respiration guided treatment technique. Using a gated dose delivery approach it was possible to increase the mean dose inside the target volume from 90% up to 99% of the prescribed dose.
