Abstract

Im Rahmen dieser Arbeit wurde der Prototyp eines Nadelhalters für perkutane Interventionen an MR-Tomographen entwickelt. Mittels kleiner, am Nadelhalter angebrachter aktiver Markerspulen ist dieser durch Projektionstechniken lokalisierbar und es ist möglich, die Schichtorientierung für die Bildgebung automatisch einer bewegten Nadel nachzuführen. Zur Darstellung der Nadel, die nur indirekt durch ihr Artefakt sichtbar ist, wurden schnelle MR-Pulssequenzen (trueFISP, HASTE) implementiert, die den Eingriff in nahezu Echtzeit darstellen können und die Artefaktgröße im Vergleich zu konventionellen Gradientenecho-Sequenzen verkleinern. Zur Nadelverfolgung wurde eine neuartige targeted-HASTE-Sequenz (TASTE) entwickelt, die es ohne Einfaltungsartefakte ermöglicht, einen schmalen Streifen um die Nadel abzubilden. Im Vergleich zu konventionellen HASTE-Sequenzen konnte dadurch sowohl die räumliche Auflösung als auch die Bildwiederholrate gesteigert werden. Im Tierversuch wurde die TASTE-Bildgebung in Kombination mit einer robotischen Positionierungshilfe und automatischer Schichtnachführung evaluiert. Nach vorheriger Interventionsplanung konnten mehrere Nadelvorschübe mit Hilfe der Echtzeitbildgebung erfolgreich durchgeführt werden. Ein weiterer Teil der Arbeit beschäftigte sich mit der Lokalisation von induktiv gekoppelten passiven Markerspulen. Mit einer passiven Spule, die auf die Thoraxwand von Probanden angebracht wurde, konnte die Atembewegung direkt gemessen und zur retrospektiven Bewegungskorrektur von Spinecho-Bildern genutzt werden. Mit einer schnellen Positionsbestimmung der Spule konnte in Verbindung mit abdomineller trueFISP-Bildgebung die äußere Atembewegung mit der inneren Organbewegung korreliert werden. Im Rahmen einer klinischen Studie wurde diese Methode genutzt, um die Bewegung von inneren Organen bei verschiedenartigen Atemmanövern zu untersuchen. Diese Methode soll die Grundlage darstellen für ein verbessertes Behandlungskonzept der Strahlentherapie bewegter Tumore.

Translation of abstract (English)

In this work a prototype needle holder was developed for percutaneous interventions in MR-scanners. Using small active coils attached to the needle holder its position could be detected by projection techniques and, thus, slice orientation for imaging could automatically be tracked the needle motion. For visualization of the needle only visible by its artefact fast MR pulse sequences (trueFISP, HASTE) have been implemented, which can image the intervention in near real-time and which can reduce the size of the artefact in comparison to conventional gradient echo sequences. For needle tracking a novel targeted-HASTE sequence (TASTE) was developed, which allows for imaging of a small strip in the vicinity of the needle without aliasing artefacts. Compared to conventional HASTE sequences both spatial resolution and image frame rate could be increased. In an animal experiment the TASTE imaging with automatic slice positioning has been evaluated in combination with a robotic assistance system. After intervention planning several needle insertions were successfully performed using real-time imaging. Another part of this work dealt with the localization of inductively coupled passive marker coils. Using a passive coil fixed on the thoracic wall of a volunteer, the respiratory motion could directly be measured and could be utilized for a retrospective motion correction of spin echo images. With the fast localization of the coil in combination with abdominal trueFISP imaging the external motion could be correlated with the internal organ motion. In a clinical study this method was used to explore the movements of inner organs due to different breathing manoeuvres. This method has the potential to improve treatment concept for the radiation therapy of moving tumors.
