Abstract
Ziel dieser Arbeit war es, einen Micro-Multileaf-Kollimator (Micro-MLC) als Beispiel für ein Mikroblendensystem hinsichtlich seines möglichen Einsatzes in der intensitätsmodulierten Strahlentherapie (IMRT) zu untersuchen. Anhand einer retrospektiven Patientenstudie wird gezeigt, dass der Micro-MLC wegen seiner geringen Lamellenbreite von 1,6 mm im Isozentrum anderen am DKFZ in der IMRT eingesetzten Systemen mit geringerer räumlicher Auflösung von der Therapieplanungsseite her überlegen ist. Eine ausführliche dosimetrische Untersuchung des Kollimators wurde, insbesondere auch wegen seines nichtfokussierenden Lamellendesigns, durchgeführt. Der Bereich konstanter Feldgröße beträgt für das untersuchte System 4 cm², der Bereich konstanter Dosisverteilung 1 cm² in unmittelbarer Umgebung der zentral Achse. Die grundsätzlichen dosimetrischen Eigenschaften haben sich bei Bestrahlungen auf der Grundlage eines IMRT-Planes in ein Phantom bestätigt. Ein klinischer Einsatz des Micro-MLC in der IMRT ist unter derzeitigen Bedingungen noch nicht möglich, da ein genügend genauer Dosisberechnungsalgorithmus, der die speziellen Eigenschaften des Micro-MLC während der inversen Planung berücksichtigt, noch fehlt.
Translation of abstract (English)
Object of this study was to test a micro multi-leaf collimator (MLC) as an example of a micro collimation system for the use in intensity modulated radiation therapy (IMRT). A retrospective treatment panning study showed that the micro MLC with a leaf width of 1.6 mm in the isocenter outclasses systems with lower resolution in use at DKFZ. An extensive dosimetric examination, in respect to the non focussing leaf design of the collimator, was done. As a result the region of constant field sizes for the system showed to be 4 cm² while dose rate decreases outside a 1 cm² area around the central axis. Dosimetric characteristics found have been proven by IMRT plan delivery to a phantom. The clinical application of the micro MLC for IMRT was found to be not possible presently due to the lack of precise dose calculation algorithm taking dosimetric properties of the micro-MLC into account during invers treatment planning.
